​What’s the difference between an MFT (or LMFT), an LPC (or LPCC), and an LCSW?

Licensure itself is still relatively recent history. For instance, both Marriage and Family Therapists (LMFTs) and Licensed Professional Clinical Counselors (LPCCs) did not achieve nationwide licensure until 2009, when Montana granted LMFT licenses and California approved LPCCs.

Before that time, both groups worked state by state to establish separate legal recognition, consistently emphasizing that each profession represented a unique history, body of knowledge, and skill set.

To illustrate how these differences show up in practice, consider an example: Diego, a 38-year-old Latino man who is married, works in a bookstore, and seeks therapy because of severe anxiety. How might the various disciplines frame his struggle?

Psychology
Though psychologists are not the main focus here, their perspective helps set context. A traditional psychologist would concentrate on Diego’s inner life, looking for the origins of his symptoms. Depending on theoretical orientation, this might mean exploring his childhood experiences (in a psychoanalytic approach) or his current behavior patterns (in a behavioral approach). The central focus is Diego as an individual, with attention to pathology and dysfunction.

Professional Clinical Counseling
Counseling as a profession evolved out of school and vocational counseling. Today’s LPCCs focus on mental health but still maintain that developmental perspective. They would view Diego’s anxiety as part of his personal growth and life stage. Treatment would address his psychological and social development, aiming to improve overall wellness as well as alleviate symptoms.

Clinical Social Work
Clinical social workers typically emphasize resources. These may be internal—such as coping skills or strengths Diego has not fully tapped—or external—such as support groups or community services. From this lens, his anxiety may stem from insufficient resources, and therapy would focus on connecting him with what he needs to regain balance and reduce distress.

Marriage and Family Therapy
LMFTs interpret behavior within relational systems. They might ask whether Diego’s anxiety stems from workplace dynamics or family stress. They may even see his anxiety as adaptive—for example, if it elicits care or support from his partner or boss. Since no behavior occurs in isolation, LMFTs often involve significant relationships in treatment, working to reduce the need for symptoms by changing interactional patterns.

Shared Ground
None of these approaches is inherently superior. Each shapes the training and worldview of its professionals, but in practice, there is significant overlap. For instance, LMFTs and LPCCs may handle child adjustment issues in very similar ways. Over time, the disciplines have also influenced one another—leading to fields like Community Psychology (similar to social work), Family Psychology (similar to MFT), and Counseling Psychology (akin to LPCC).

This cross-influence enriches all professions and broadens clinical perspective. Still, differences remain. For example, if we look at LMFT competencies, they can be grouped into three categories: Skills every mental health provider should have and would apply similarly (such as suicide risk assessment).

Skills shared across professions but approached differently by LMFTs (such as general assessment, which MFTs do with a relational lens). Skills uniquely emphasized in MFT training (such as systemic case conceptualization). The same breakdown could be applied to the competencies of LCSWs or LPCCs. While the professions often do similar tasks—assessing, diagnosing, and treating mental illness—it is misleading to suggest they are interchangeable. Their underlying frameworks matter, shaping both how they understand clients and how they intervene.
